EDS Awareness is pleased to be invited to attend the International EDS Symposium (EDSI) in New York City, May 3-6, 2016. There will be more than 250 participants, including doctors from all over the world.
We have offered to provide the results of a comprehensive survey of patients with Ehlers-Danlos Syndrome and associated conditions. The data collected from this survey will be analyzed and
included in our upcoming report to be distributed at the symposium. Your participation will help us to educate patients, families and healthcare providers.
Our Survey Team of EDS Support Group Leaders have developed this survey and ask that you take a few minutes out of your busy day to help us provide "Feedback to the Doctors".
Survey Details:
A. Please complete this survey by Tuesday, April 5, 2016.
B. Please share the survey link on your social media/Facebook, local
support group websites, and email it to other EDS patients within your network.
D. Typing is minimal. The survey consists of 50-60 questions (depending on your unique patient experience). The majority are “check-the-box” answers.
E. The survey can be answered in less than 30 minutes.
F. You may take breaks. At any point in time before completing the survey, you can exit by closing the
window. Your incomplete results will be saved automatically and you can re-open your survey through the link - as many times as you need to - until you click DONE.
